Transcendent Care, LLC sees the lack of available supported living services for individuals with developmental disabilities as a serious issue that needs solving. There are approximately 1000 individuals currently on the state Department of Mental Health waiting list for these critical services, as well as hundreds of children in state custody with no place to go. Without the life skills training, stable housing, health and behavioral support that supported living services provides, many of these individuals end up in hospitals, homeless or incarcerated.
The challenges these individuals face are complex, and servicing them requires highly trained staff with expertise in areas like behavioral support, mental health, trauma, and crisis management. There is a shortage of providers with the skill set to support individuals with the most severe behavior challenges. Transcendent Care will leverage state-of-the-art training, strict vetting procedures and data-driven best practices to close this gap, allowing more highly challenged individuals to access life-changing independent supported living services.
In providing these services, Transcendent Care will help address Missouri’s shortage of supported living opportunities and give those with the greatest needs a chance at safe and fulfilling lives in their communities. Our model is designed specifically to serve this underserved population, and partnerships with local organizations will help identify those most in need of intervention and ongoing support.

A cornerstone of our solution is community integration. We facilitate participation in social and recreational activities, as well as access to educational services for children. Comprehensive treatment plans that emphasize behavior reduction, skill acquisition and personal choice are developed for each individual served. These plans are based on formal assessments of needs, values and skills.
Transcendent Care maintains a team of skilled nurses, direct support professionals and house managers to provide medication management, responsive care, and coordination of medical and mental healthcare needs. Most importantly, we are dedicated to giving individuals served choice and control over their lives while maintaining a safe living environment.
The target market for Transcendent Care is individuals with developmental disabilities that exhibit significant behavioral challenges. This includes individuals diagnosed with autism, Down syndrome, cerebral palsy, and other developmental delays that require specialized care.
According to the Department of Mental Health, there are currently over 1,000 individuals in our state waiting for placement in supported living services. Many of these individuals have been waiting for years due to a lack of providers able to support their needs.
In addition, the Children's Division has reported hundreds of children currently awaiting placement, with some even having to sleep in case workers' offices because there is nowhere else for them to go. These children often have extreme behaviors that most providers cannot handle.
We have existing relationships with case coordinators who have referred potential clients to us that are ready to begin services as soon as we open. Based on the sheer number of individuals awaiting services in the state and our specialty in supporting those with extreme behaviors, we are confident that we will reach full capacity quickly.
Our competitive advantage lies in our extensive experience and specialized training in managing significant behavioral challenges. We offer a unique solution that does not currently exist for many of these individuals and families in need.
There are 51 providers in the Eastern Region of Missouri and 19 of those are located in the Greater St. Louis area that offer supported living services. However, as evidenced by research studies and reports, there are not enough providers and residential placements to support the current population in need of these critical services.
A recent article indicated there are 650 adults with developmental disabilities living in inappropriate placements in Missouri, including hospitals and jails. There are also over 1,500 children in the custody of the Missouri Children's Division residing in residential placements. Due to COVID-19, Missouri lost about 50% of its residential beds for individuals with developmental disabilities.
The lack of appropriate housing and support services means many children and adults with developmental disabilities and behavioral challenges end up living in hospitals, jails, and other unsuitable settings with little hope of transitioning to more appropriate care. This illustrates the tremendous unmet need for high-quality supported living providers in Missouri, especially in the St. Louis region.
Transcendent Care is primed to help address this shortage and provide an alternative to the limited options currently available to individuals with developmental disabilities in the area. By offering personalized supported living services focused on community integration, relationships, and maximizing independence, Transcendent Care will provide a life-changing option for individuals currently struggling without necessary aid and support.

Owning residential properties will allow us to offer choice in housing to those we serve, complying with Home and Community Based Services regulations.
The adaptive homes we acquire and modify will likely require the following key equipment and tools:
- Reinforced walls and floors to withstand damage and environmental changes
- Unbreakable windows for safety and security
- ADA-compliant modifications such as ramps, grab bars, and widened doorways to improve accessibility
- Fire suppression systems to ensure quick emergency response
- Integrated technology such as automated medication dispensers, smart home features, and emergency alert systems tailored to clients’ needs
The specific equipment and tools required for each home and level of modification will depend on a client’s wants, needs, and risks identified in their individual service plan. However, for any home acquired, we aim to utilize high-quality, durable, adaptive, and integrated equipment to offer clients supportive, independent, and safe housing. With these kinds of investments and modifications made upfront, we believe clients can have stable long-term housing and staff will be better equipped to support clients’ needs.
