Healthcare workers witness safety risks, system failures, and near-miss events every day—but the vast majority of these incidents are never formally documented or acted upon. Existing reporting systems inside hospitals are often slow, bureaucratic, or intimidating to use, and many clinicians fear retaliation for speaking up. As a result, critical patterns—such as unsafe staffing ratios, equipment failures, medication errors, or workflow breakdowns, short staffing, ADO reports, and missed breaks—frequently go unnoticed until serious patient harm occurs.

The platform we are developing is a secure, independent reporting and analytics system designed specifically for frontline healthcare workers. Through a simple mobile interface, nurses, physicians, and other clinicians can quickly document near misses, safety concerns, and system issues in real time. These reports are anonymized, aggregated, and analyzed to identify patterns across departments, hospitals, and healthcare systems. Instead of isolated complaints, the platform transforms individual observations into large-scale, actionable data that highlights systemic problems before they lead to adverse events.

Beyond reporting, the platform provides value to users and organizations through actionable insights. Healthcare workers gain access to safety trend dashboards (when job seeking), benchmarking data, and educational insights based on real-world clinical patterns. Hospitals and healthcare organizations can use anonymized aggregated data to proactively address risks, improve workflows, and strengthen patient safety programs. Over time, the platform will create one of the largest datasets on frontline healthcare safety issues—information that can support research, policy reform, and quality improvement initiatives nationwide.

Our long-term vision is to create a national safety intelligence network for healthcare, similar to how aviation tracks safety events to continuously improve systems. By empowering clinicians to contribute data safely and easily, we can move healthcare from a reactive model—responding to errors after they occur—to a proactive system that identifies risk patterns early and prevents harm.
