Town Square Innovative Adult Day is redefining dementia care by helping older adults remain safely at home while providing meaningful support to family caregivers.
Town Square Innovative Adult Day is an RN-owned and operated adult day health and dementia care center serving older adults, individuals living with dementia, and their family caregivers throughout the Tri-Cities, Washington. Our mission is to help older adults remain safely at home longer by providing exceptional daytime care, nursing oversight, therapeutic programming, and meaningful support for family caregivers.
What sets Town Square apart is its immersive reminiscence environment, thoughtfully designed to resemble a nostalgic mid-century American town. Members enjoy familiar spaces including Rosie's Diner, a vintage movie theater, Glenner Park, and other familiar settings that encourage conversation, stimulate memories, promote independence, and create joyful daily experiences. Combined with evidence-based dementia care, individualized programming, exercise, music, art, nutritious meals, and RN oversight, the environment is designed to improve quality of life while reducing isolation and caregiver stress.
We serve families seeking a safe, engaging alternative that allows their loved ones to remain living at home while receiving high-quality daytime care. Our referral partners include healthcare providers, hospitals, case managers, senior living communities, home care agencies, and community organizations that recognize the value of coordinated, community-based care.
Town Square currently accepts private pay, Medicaid, and PAVE funding and is actively completing the approval process to serve eligible veterans through VA funding. As an established healthcare provider with growing community awareness and expanding referral relationships, we are positioned to meet the increasing demand created by an aging population and the rising prevalence of dementia. Recent enrollment trends, expanding healthcare partnerships, and growing community awareness continue to validate the demand for our model.
The company is led by Registered Nurse Mandy McCollum, who has spent her career improving the lives of older adults through senior care, dementia education, healthcare operations, and caregiver training. Her vision has shaped a culture centered on compassion, clinical excellence, and innovation in aging services.
