Community Health and Urgent Care, LLC

Hi, I am a female Nurse Practitioner who has opened a rural health clinic that closes the gap of healthcare disparities and inequality by serving several rural communities within a 30 mile radius with:
 in office primary care, urgent care, and home visits for the  elderly unable to come into the office setting.  This medical office will also target the opioid crisis that is affecting every part of our nation,  specifically in the rural  areas within the Pee Dee and Midlands region of South Carolina.  

I have personally seen the devastation  that the opioid crisis has on our young and old populations alike without discrimination,  and these are business professionals, lawyers, medical personnel,  students, moms, dads and children who are losing their battle daily to this addiction. I have been treating patients with the Suboxone program since 2018 in a collaborative effort with The Rubicon Center in Hartsville, SC and making a huge impact within our community.  

I have been a Family Nurse Practitioner since 2014, this is my ministry. I work with patients from age 4 days old to over 100 year old.  I manage acute and chronic medical conditions as well as the Suboxone program.  I have a good patient base that could be expanded greatly if I have the office location.  I am personally credentialed with  all major insurance companies and will only need to complete the credentialing for my business. 

I have a Cardiologist that has committed to being in the office weekly to see all patients with cardiac needs and I have a Pediatric MD that will be in the office weekly to see pediatrics for behavioral health.  I have a team of psychologist and counselors to address behavioral health via telehealth or in person visits at their office.  

This is a dream that has been instilled in my heart and a much needed service within our community that can be very profitable as well.  I need funds to lease the building, renovations, equipment purchases and general start up funds. I am currently making most all home visits at this time and staying busy.  Any consideration would be greatly appreciated.
